Case Management
     
  Defintion:
   

Case management is a collaborative process of assessment, planning, facilitation and advocacy for options and services to meet an individual's health care needs through communication and available resources to promote quality cost-effective outcomes.

The defintion above was modified in 2002 as outlined in the revised 2002 Standards of Practice for Case Management.
     
  Objectives:
    To provide continuing education and information to enhance the skill sets of case managers and affiliates.
    To promote optimal case management practice for case managers.
    To foster professional growth and development within a supportive and friendly environment.
    To promote the highest standard of professional practice in case management.
    To promote the highest standard of ethical practice within case management.
